  10.  AB    Azole-derived antifungal medications are particularly involved in
  11.        drug-induced hepatic injury encountered in patients with AIDS.
  12.        Fluconazole may induce multiple hepatic abnormalities usually
  13.        characterized by asymptomatic and reversible mild hepatic necrosis. We
  14.        here describe severe subacute liver damage occurring in a patient with
  15.        AIDS who was receiving fluconazole maintenance therapy for a
  16.        cryptococcosis. Hepatotoxicity was essentially characterized by mixed
  17.        cytolytic and cholestatic liver tests abnormalities which improved after
  18.        fluconazole discontinuation and worsened on fluconazole rechallenge.
  19.        Optical microscopy demonstrated nonspecific abnormalities including
  20.        granular aspect of the cytoplasm of the hepatocytes. In contrast,
  21.        analysis of electron microscopy revealed unusual unreported features
  22.        characterized by giant mitochondria with paracrystalline inclusions and
  23.        enlarged smooth endoplasmic reticulum. All microscopic abnormalities
  24.        were reversed after discontinuation of fluconazole. We suggest that
  25.        persistent increased hepatic enzymes in HIV-infected patient taking
  26.        fluconazole should prompt suspension of the treatment. Prospective
  27.        studies are needed to determine whether careful monitoring of hepatic
  28.        tests should be recommended in AIDS patients on prolonged fluconazole
  29.        maintenance therapy.
